Using the Tripod Beta Methodology, we investigate incidents in three steps. With these three steps, we help organizations to identify and eliminate operational deviations that precede accidents and incidents. This is the main purpose of Tripod and therefore it is also a powerful model for investigating underlying causes. The method is explicitly not aimed at apportioning blame for an incident.
