Because we want to help prevent incidents from happening again. And this is only possible if we fully understand how they came about. Our experience shows that (major) damages are often caused by an - often minor - technical cause. Unfortunately, human behaviour plays an important role in almost every damage caused by an incident.
It is therefore important to discover why the incident was able to happen. Learning from (near) accidents and incidents is absolutely necessary to prevent similar incidents from happening again.
All too often, however, investigations do not go far enough in identifying failed safety features. What is particularly lacking is a thorough investigation of human factors and the exposure of organisational weaknesses. And that is exactly where we can help.
